Dayton Sports Medicine Institute will accept individual cancellations with a full refund of course fees up to one week prior to scheduled event. No refunds will be made for cancellations less than 7 days till the event. No refunds will be made for no-shows on day of scheduled event. In order to process refunds, individuals must fill out a W-9 form.
