Physicals Sign Up for May 8th 2019
Dayton Sports Medicine Institute will once again be holding two nights of sports physicals for student athletes at DSMI affiliated Preble County Schools. Student athletes registering for sports physicals must be going into grades 7-12 next year. The cost will be $10 per athlete paid by cash or check the night of the sports physical. Registration will be done by choosing one of the four time slots below, entering your valid email address followed by name, grade and school. Please arrive at your designated time on the night of sports physicals to help avoid long lines. Please bring your completed physical form with you with parent/guardians signature. These physicals would not be possible without the collaboration of Dayton Sports Medicine Institute, Kettering Health Network, Orthopedic Associates of SW Ohio, Preble County Chiropractic’s, Preble County Medical Center and the Primary Care Physicians within the building. Please enter through the Physician's office door. Signs will be up around the medical center to help direct you to the correct location.
WHAT TO BRING WITH YOU:
Please avoid drinking caffeine or exercising right before coming to physicals. This can skew test results and could cause you to need to be referred for follow up care.
REFERRAL FOR FOLLOW UP CARE:
Please be aware that participating in Sports Physicals may still result in a recommendation or requirement for follow-up care. Recommended or Required follow-up care is done in the best interest of the health and safety of your student athlete. Diagnosis that may need further care would be, but not limited to;
Early detection and treatment of injuries and illnesses is imperative to the lifelong health of your student athlete. If recommendations or requirements are needed post sports physical a form will be provided on what is needed.
